Description
40 hours per week, M-F 8-4:30pm. CHPN Certification Preferred. Team consists of 4 doctors, 1 clinical nurse manager, 5 full time nurses and 1 per diem nurse, 1 social worker and 1 chaplain. Hospital has 523 beds. Adult and Pediatric Palliative Care Team. Independently assess and monitor of the physical, emotional, and spiritual status of patients and families who are dealing with advanced illness. Works collaboratively with an interdisciplinary team of healthcare professionals to manage the patient's care. Educates patients and their families on the nature of the illness, the progression of the disease, and end-of-life care when appropriate. Provides thorough assessment of a patient's goals of care and guidance for healthcare decisions, including advance care planning and ethical issues. Coordinates resources and services available to both the patient and their family to support them along the disease trajectory. Participates in regular reviews of the patient's care plan to ensure it is meeting the patient's needs and aligned with their goals. Coordinates referrals to other healthcare professionals and services as needed, including education about hospice services when appropriate. Delivers holistic and individualized care to patients in all areas of the hospital. Develops, implements, and coordinates a plan of care that incorporates psychological, sociocultural, spiritual, economic, and life-style factors. Fosters and maintains collaborative relationships between patients, their family/support group, physicians, and other healthcare providers through timely and effective communications. Collaborates with social workers, chaplains, case managers, and outside agencies to coordinate care. Adheres to hospital policies, industry standards, best practices, and applicable laws and regulations to promote an optimal patient experience.
